Eligibility

Class members are individuals residing in the United States whose personally identifiable information the ACPHS data security incident that occurred between Aug. 31, 2024, and Sept. 14, 2024, potentially impacted, including those who received notice of the incident. How much are settlement payments? Class members have the following benefit options: Reimbursement for documented losses : Class members with out-of-pocket losses traceable to the data security incident may qualify to claim up to a maximum of $5,000 per person.
